All Public Records Requests must be made in writing and submitted to the Public Information Officer either in person at 1020 Henry Clay Street, Shelbyville KY or by email to Todd Martin ([email protected]). 

All requests to the North Central District Health Department concerning open records will be handled in accordance with KRS 61.870-61.884, the Open Records Act.

All request submissions will need to be completed on the state’s Standard Open Request Form provided by the Kentucky Office of the Attorney General.
